POSITION REQUIREMENTS: Licensed Registered Nurse in the State of Michigan.  BSN or BS in healthcare preferred.  Must have minimum of two years in home hospice experience. Experience with case management of hospice patients. Excellent communication skills. Ability to use computer for documentation. Dependable transportation. Ability to work well with others in stressful situations. Team player. Knowledge of pain management techniques.     

POSITION RESPONSIBILITIES: Case management of home base hospice patients. Assessment of patient and family needs. Communicate with team as needed. Attend weekly IDT meetings. Facilitate patient/family goals through interdisciplinary relationships.

This position is 40 hours a week.  Rotating weekends and on-call as well.
